Team Check In

The State of Ohio requires coaches, players and parents to be aware of the risk of Sudden Cardiac Arrest and Concussions in player participation sports. 

Coaches are required to review information and complete the CDC Concussion training  and watch the Sudden Cardiac Arrest video available at the links below.

The following Concussion information and Sudden Cardiac Arrest Information is required to be distributed to be distributed to parents of players participating in our event.

Team online check-in must be completed one week prior to the start of the event.

Please upload the following documents using your team's Gotsport online account.

Roster: Copy of your team’s official roster. Please cross off players who WILL NOT be attending. 

Guest Player Roster: USYS teams utilized USYS guest players must upload a copy of the guest roster for our event.

Player Pass Cards: Copies of validated player/coach/manager passes issued by USYS or US Club Soccer (rostered players and guest players) as they are listed on your tournament roster. All pass cards must be issued by the same USSF organization.

Guest Player Pass Cards: Copies of validated guest player pass cards issued by the same USSF organization as the participating team.  If adding guest players within 72 hours prior to the start of the tournament, you must notify the event director at [email protected] tournament staff il that you added a guest player so that player can be approved. You must also submit/upload the guest player’s player pass online 1 hour prior to the start of your first game.

Permission to Travel: USYS Teams traveling outside the state of Ohio for our event must obtain this form from their state association.  US Club Soccer teams teams do not need a permission to travel document.

Medical Releases: Completed medical release forms for each player (including guest players) must be in the possession of the team representative for each match.  Medical release forms DO NOT need to be uploaded to your Gotsport team account.

Upload Confirmation:  You will only receive communication if there are issues with your check-in documents.
